Report a Concern
Choose from the options below to find out where to report a concern or issue you are experiencing at UC San Diego.
Please note that this page primarily includes resources for policy-related and compliance concerns. For general assistance with other issues (such as technical or HR support), please visit the UC San Diego Services & Support portal.
For confidential assistance with dispute resolution, mediation, or connecting to appropriate university resources, contact the Office of the Ombuds.
If you are experiencing an emergency or an imminent threat to health or safety, call 911.
Abusive conduct (intimidation, harassment, abusive language, and similar behavior)
You can report abusive conduct via the Abusive Conduct Reporting Form. To learn more about what kinds of behavior are considered abusive conduct under UC policy, you can read the UC Abusive Conduct Policy and take the UC Abusive Conduct in the Workplace training (required for all employees).
Academic integrity violations or non-academic student misconduct
You can report these concerns using the web form for academic integrity violations and non-academic student misconduct. Find more information about academic integrity at the Academic Integrity Office website, and learn more about student conduct processes at the Center for Student Accountability, Growth, and Education website.
Child abuse or neglect
Report child abuse or neglect according to the California Child Abuse and Neglect Reporting Act (CANRA). See UC San Diego's Reporting Child Abuse and Neglect Procedures and Safe Supervision of Minors at UC San Diego for more information.
To make a report:
- Call the Child Abuse Hotline at (800) 344-6000 / (858) 560-2191 and one of the following local law enforcement agencies:
- UC San Diego Police Department: 911 or (858) 534-HELP; or
- San Diego Police Department: 911 or (619) 531-2000 or (858) 484-3154; or
- San Diego County Sheriff’s Department: 911 or (858) 565-5200
- AND Submit Form 8572 within 36 hours to the County of San Diego, Child Welfare Services, HHSA Child Abuse Hotline using one of the following three methods:
- Fax to: (858) 467-0412
- Mail to: Child Welfare Services/Hotline, 8911 Balboa Ave, San Diego CA, 92123; or
- Online: via a web-based system link through the MRA (Mandated Reporter Application), which you will receive after making your phone report to the Child Abuse Hotline. See the San Diego County District Attorney Mandated Reporting page for more information.
- AND Report observed or suspected Child Abuse or Neglect internally to the University of California Whistleblower Hotline or to your supervisor. If reported to a supervisor, the supervisor must make the report to the University of California Whistleblower Hotline:
- By telephone: (800) 403‐4744; or
- Online:University of California Whistleblower Hotline
The report may be anonymous; however, providing contact information for the Mandated Reporter may expedite the University’s ability to follow-up on the report.
Clery crime reporting for Campus Security Authorities
If there is an active crime, emergency or a serious or continuing threat, call 9-1-1.
- Report a Clery crime as soon as possible to the UC San Diego Police Department. Remember, a report is required if a Campus Security Authority witnessed the crime or it was reported to them.
- File a Clery report online or complete a fillable PDF and send it to csareport@ucsd.edu.
- Ask the victim/survivor for their permission before providing personally identifiable information on the Clery report.
- If the incident involves dating violence, domestic violence, sexual assault, stalking, or a hate crime/bias incident, you must also report the incident to the Office for the Prevention of Harassment & Discrimination if required by Title IX/UC Policy.
- As a CSA, never try to investigate or prove a crime actually occurred.

Fraud, corruption, misuse of university resources, or significant threats to health or safety
UC San Diego is committed to maintaining a community where faculty, staff, and students are free to raise good faith concerns regarding the University's business practices, and we encourage such reporting. UC policies prohibit retaliation against any person who, in good faith, has reported a suspected violation of law or policy.
Unlawful acts such as corruption, bribery, theft or misuse of University property, fraudulent claims, fraud, coercion, willful omission to perform duty, economic waste, gross misconduct, gross incompetence or gross inefficiency; or any condition that may significantly threaten the health or safety of employees or the public should be reported as described in the UC Whistleblower Policy. These concerns should be reported through the UC Whistleblower Hotline online form or by calling the hotline at (800) 403-4744.
Harassment or discrimination based on race, religion, age, or other protected class
Report these concerns to the Office for the Prevention of Harassment & Discrimination (OPHD) via the OPHD Webform, via email at ophd@ucsd.edu, or by calling (858) 534-8298.

Research misconduct
To report concerns about research misconduct, please contact Corinne Peek-Asa, PhD, Vice Chancellor for Research and Innovation, Research Integrity Officer, at researchintegrity@ucsd.edu. Safety concerns (non-emergency)
You can report a safety concern that is not an emergency or a “near miss” (conditions or events with the potential for serious consequences to safety) through the Environment, Health & Safety Concern Form or by calling EH&S at (858) 534-3660. You can choose to remain anonymous in your report, although it is recommended that a name, phone and email be provided so a safety expert can better understand and resolve the issue.

Sexual harassment, sexual violence, or other prohibited conduct
Report these concerns to the Office for the Prevention of Harassment & Discrimination (OPHD) via the OPHD Webform, via email at ophd@ucsd.edu, or by calling (858) 534-8298.
Please note that UC employees (including faculty) are required to report sexual violence, sexual harassment, or other behavior prohibited by the university’s Policy on Sexual Violence and Sexual Harassment to the Title IX office if this behavior involves a student (on our campus, the Title IX office is OPHD). They are encouraged to report these behaviors involving anyone affiliated with the university.
Workplace violence
Any acts of workplace violence or threats indicating an imminent risk of workplace violence should be reported directly to the UC San Diego Police Department. Emergencies or situations that may pose an immediate or ongoing threat to the health and safety of our students, employees and visitors should immediately be reported by calling or texting 9-1-1, or by using one of the call boxes located in the parking areas. For non-emergencies, please call (858) 534-HELP (4357).
The UC San Diego online Incident Reporting Portal allows employees to report incidents of violence or threats without fear of retaliation. Reports can be made through the UC San Diego Workplace Violence Incident Reporting Portal.
